Integrating community health organizations into your enterprise

Allow community partners with external systems to easily share patient records and make appointments in your Epic. This enables time critical workflows such as tele-stroke, electronic consults, and test-to-treat to be completed using each provider's native EMR.

ExtendedCare customers regularly work with federally qualified health centers (FQHCs) to help improve the quality of life for their underserved patients. By allowing providers at these external community organizations to engage in on-demand video visits using the call queuing system built into the ExtendedCare Provider Portal, remote providers have an extremely simple workflow in which they can focus on delivering care and extending the reach to more patients that need help; without the overhead of needing to be onboarded into the EHR system.

Staff at the external clinics in turn use ExtendedCare’s intake form functionality to capture patient data and any associated notes before joining a video visit queue. Each participant waits in their own unique virtual waiting room where content such as YouTube videos, digital signage information, and many other forms of engaging content can be presented to them until their provider joins.
